OrthoCare Pain and Rehabilitation Medicine was founded in 2018 as the need for physical medicine and rehabilitation physicians (physiatrists) in the post-acute care setting was evident. As the acuity of the patient demographic in that setting changed, the need for more comprehensive care in it grew. Rehabilitation centers are continually being evaluated on quality of care, readmission rates and patient satisfaction. OrthoCare collaborates with these rehabilitation centers and medical attendings to improve quality of care, coordinate treatment and manage rehabilitation needs within the rehabilitation center.
Our Services
Physiatrists complete core competencies during residency that provide us with a skill set to improve function and decrease pain within post-acute care settings. We treat the whole patient and formulate individualized comprehensive treatment plans. These services include performing bedside injections (joint, trigger point) that a patient would typically have to leave the facility to have performed. As maximizing function is our primary goal, we also prescribe appropriate durable medical equipment, orthotics and prosthetics that will help improve performance in therapy and in the community upon discharge.

Our goal is to establish a rehabilitation presence in skilled nursing facilities (SNF). We want the patients to know they have a provider tracking their progress during their stay. SNF benefit from our presence because they have an in-house provider to address medical rehabilitation concerns and to provide appropriate documentation to meet compliance requirements.
